NOTE: The Service Delivery Act provides DCA with 30 days to review SDS documents to verify their compliance.  Be aware that all deadlines are the dates by which DCA must have verified that your SDS is compliant--the deadlines are not submittal dates, they are process completion dates.  We recommend 60-90 day lead times to ensure that reviews can be completed and any necessary revisions conducted and subsequently reviewed before passage of applicable deadlines.

These are the standard forms that, when properly completed, comprise a countywide Service Delivery Strategy (SDS).  Please use these SDS Forms to guide you through the process of renegotiating your service delivery strategies and transmitting them to DCA for verification.

  • SDS FORM 1:  Provides general instructions to the local governments for how to use the other forms.  It also acts as a cover for the rest of the document providing an overview of the parties to the SDS, the services covered in it, and which services are being extended without change or are being modified.  Your SDS must include one copy of the FORM 1.
  • SDS FORM 2:  Details the mechanisms for local service delivery describing which local governments/authorities provide what services to which territories and how they're funded.  A FORM 2 for each service being added to the SDS, each service with modified delivery arrangements, and each service being described/explained differently must be included in the SDS.
  • SDS FORM 3:  Presents the any incompatibilities in local land-use plans which were discovered in the process of creating the new SDS and how they were resolved.  It also discusses the mechanisms that local governments will use to resolve conflicts over service delivery, particularly the extension of extraterritorial water and sewer services.  The FORM 3 must be included in any SDS when it has been 10 or more years since the last FORM 3 was completed.
  • SDS FORM 4:  Is completed with the names, titles, dated signatures of each of the required signatories.  It includes a list of certification statements that the signatories agree to in order to formalize a modified SDS.  FORM 4 must be included in any SDS Update that necessitated inclusion of any FORM 2s.
  • SDS FORM 5:  Is completed with the names, titles, dated signatures of each of the required signatories.   It includes a list of certification statements that the signatories agree to in order to extend an existing SDS, as-is, with no modifications.  It can only be used if it was unnecessary to include any FORM 2s in the updated SDS.

REQUIRED SIGNATORIES:  SDS FORM 4 or 5 must, at a minimum, be signed/dated by the chief elected official of the following governments: 1) the county; 2) the city serving as the county seat; 3) all cities having a 2020 population of over 9,000 residing within the county; and 4) no less than 50% of all other cities with a 2020 population of between 500 and 9,000 residing within the county.  Cities with a 2020 population below 500 and local authorities providing services under the strategy are not required to sign this form, but are encouraged to do so.
