ALTERNATIVE DISPUTE RESOLUTION & ANNEXATION ARBITRATION FORMS

Use these interactive forms for all work relating to the ADR processes required for the resolution annexation disputes and other interjurisdictional conflicts.  DCA will not accept forms that deviate in format from the files on this site. SDS BUILDER (Service Delivery Strategy Forms)

The SDS Builder provides local governments the ability to create and update their Service Delivery Strategies online. Beginning in August 2011, it replaces the the former paper-driven system. This database is password-protected, and is currently available only to local governments and Regional Commissions.  Future database improvements  will include a guest portal for all users wanting to review existing Service Delivery Strategies.

To access online forms, CLICK HERE.  (Please note:  to complete SDS forms through this site, county users MUST have the password issued by DCA. In most cases, the county clerk for the jurisdiction will have access to the government's password.)
